Who is eligible?
To be eligible for IMPACT WV, a mother must:
- Live in Marion, Harrison, Preston, Ohio, Marshall, Wetzel, Taylor, Tyler, or Monongalia Counties
- Have a child diagnosed with Neonatal Abstinence Syndrome (NAS) or exposed to substances in utero who was born in the last 3 months
- Be an active client in a Home Visiting Program no later than the infant turning 3 months of age
What is a Patient Navigator?
The WV IMPACT Project Navigator will be responsible for coordinating the care of multigenerational families impacted by substance use and NAS. Services will focus on meeting “where the client is,” by responding to current client needs such as food, shelter, education, clothing, transportation, and/or childcare. The three IMPACT Demonstration Sites, Wheeling Hospital Perinatal Transition Program, WV Court Appointed Special Advocates (CASA), and Burlington United Methodist Family Services (BUMFS) use Patient Navigator to coordinate services. The IMPACT WV Education and Employment Service Specialist helps to reach school or work goals, provides individualized programs based on needs and goals, and offers in person or remote support on finances, identifying career skills, education options and connections to additional services.
